Re-land LLDB dap module symbol tables (#155021)

Re-land the symbol table feature in lldb-dap after it was
[reverted](https://github.com/llvm/llvm-project/commit/2b8e80694263fb404d1d0b816f33df731e617625)
because of a crash in the `aarch64` tests, which was caused by
dereferencing `SBSymbol::GetName` which might return `nullptr` for an
invalid symbol.

This patch reapplies the original commits and adds the missing null
check.

Also adding `skipIfWindows` for the module symbols tests, since LLDB
doesn't recognize the symbols from a.out there.

This reverts commit 8b64cd8be29da9ea74db5a1a21f7cd6e75f9e9d8.

This breaks lldb-aarch64-* bots causing a crash in lldb-dap while
running test TestDAP_moduleSymbols.py

* Changes the default synthetic symbol names to contain their file
address

This is a new PR after the first PR (#137512) was reverted because it
didn't update the way unnamed symbols were searched in the symbol table,
which relied on the index being in the name.

This time also added extra test to make sure the symbol is found as
expected

[lldb] Support overriding the disassembly CPU & features (#115382)

Add the ability to override the disassembly CPU and CPU features through
a target setting (`target.disassembly-cpu` and
`target.disassembly-features`) and a `disassemble` command option
(`--cpu` and `--features`).

This is especially relevant for architectures like RISC-V which relies
heavily on CPU extensions.

The majority of this patch is plumbing the options through. I recommend
looking at DisassemblerLLVMC and the test for the observable change in
behavior.

For the significant amount of call sites that want to create an
incontrovertible error, such a wrapper function creates a significant
readability improvement and lowers the cost of entry to add error
handling in more places.

[lldb] refactor highlighting function for image lookup command (#76112)

This patch replaces uses of StringRef::{starts,ends}with with
StringRef::{starts,ends}_with for consistency with
std::{string,string_view}::{starts,ends}_with in C++20.

I'm planning to deprecate and eventually remove
StringRef::{starts,ends}with.
