To support detecting MD5 checksum mismatches, deal with SupportFiles
rather than a plain FileSpecs in the SourceManager.

[lldb] Sink StreamFile into lldbHost

StreamFile subclasses Stream (from lldbUtility) and is backed by a File
(from lldbHost). It does not depend on anything from lldbCore or any of its
sibling libraries, so I think it makes sense for this to live in
lldbHost instead.

Removing .c_str() has a semantics difference, but the use scenarios
likely do not matter as we don't have NUL in the strings.

[lldb] Decouple instrumentation from the reproducers

[lldb] Remove reproducer instrumentation

This patch removes most of the reproducer instrumentation. It keeps
around the LLDB_RECORD_* macros for logging. See [1] for more details.

I was holding off on this change until we moved to C++14 as to not have
to convert llvm::make_unique to std::make_unique. That happened a while
ago so here's the first patch for the API which had a bunch of raw
`new`s.
