	* doc/xml/faq.xml: Refresh information on _GNU_SOURCE and
	_XOPEN_SOURCE being predefined.
	* doc/xml/manual/internals.xml: Remove outdated paragraph about
	_POSIX_SOURCE in libstdc++ source files.
